It’s that time of year again to schedule children for annual sports physicals.
Children enrolled in Family Health or Pediatrics who need an annual sports physical for school should schedule an appointment through the central appointment line at (623) 856-2273 (CARE) for June 21, July 10 or 26, or Aug 7. Appointment times are between 4 and 6 p.m. each day. There is no charge associated with the physical, and appointments last approximately 15 minutes.
All forms issued by the school that pertain to the physical, as well as immunization records, and glasses or contacts are needed at the time of the appointment.
Each child will be given a sports physical which includes height, weight and vision during the provider examination. Immunizations will be verified as current.
Children with lab requirements must come in at least 24 hours prior to their appointment.
