A & B UTA (Saturday only)

Front door opens at 8 a.m. Medical records window will be open from 8 – 11:30 a.m.

 

8-10 a.m.

Fit-to-Fight (F2F) Profiles ONLY: Bring F2F Questionnaire signed by UHM, report to the Medical Group Bldg. 458

 

8-11:30 a.m.

Physical Exams (PHA) Scheduled Appointments : Report to the front desk located in the main lobby.

 

1-3 p.m.

Deployers Only: Pre/Post deployer provider visits, ANAM Testing (By Appointment)

Optometry, Gas Mask Inserts, Dental Exams, Labs Immunizations

Profiles (By appointment only)

LODs, MEBs, Reenlistments, Optometry, Gas Mask Inserts, Dental Exams, Labs

In/out-processing for TDY, Tech School, retirement, PCSs (Must have orders or letter from squadron.)

NARS members out-processing: The “Reassignment/Separation From Active Status Letter” from package

Must be signed by member’s squadron commander

Newcomers from MPF Briefing only (Report to Room 59, classroom B)

 

Weekdays (Tuesday – Friday)

Reservists must be on duty status to receive immunizations or have blood drawn. (HIV only)

 

8 – 9:45 a.m.

Provider visits by appointment only. (Report to room 18)

 

8-11 a.m.

Dental Examination Form: Bring in or fax DD Form 2813 (Personnel must call the AMDS to confirm receipt of fax.)
