A.Follow the steps below:
1. Complete the pharmacy home delivery registration form. You may register:
• Online at: https://host1.medcohealth.com/consumer/site/registration?partner=DOD&accessLink=TRICARE
• By calling 1-877-363-1303; 1-877-363-1433; TDD/TTY (for the hearing impaired) 1-877-540-6261.
• By mail: download the registration form (http://express-scripts.com/TRICARE/homedelivery/docs/dodMailOrderForm.pdf) and mail it to:

Phoenix, AZ 85072-9954
2. Place your order by asking your doctor to call Express Scripts at 1-877-283-3858 or to fax your prescription to Express Scripts at 1-877-895-1900 within the U.S. or 602-586-3911 outside the United States and its territories.
Note: By law, prescriptions for schedule II controlled substances can’t be faxed; they must be mailed.
